The agency received three reports of minor injuries including a child who was hit in the head by a chain that detached from a defective glider hanger. Owners of the defective Adventure and Create-N-Adventure play sets are being advised to remove the defective gliders immediately.

On June 12, 2008, 15,000 Playsafe Spinning Quad Merry Go Rounds sold at Toys-R-Us were recalled due to a fall hazard from a defective bearing system on the seats. The manufacturer has received one report of an injury from a fall. These defective merry go rounds were sold at Toys-R-Us from January to March 2008 for $80 to $100.
